Importance  Delayed antibiotic prescription helps to reduce antibiotic use with reasonable symptom control. There are different strategies of delayed prescription, but it is not yet clear which one is the most effective.

Objective  To determine the efficacy and safety of 2 delayed strategies in acute, uncomplicated respiratory infections.

Abstract

Background

Atopic diseases are a major burden of disease on a global scale. Regarding their aetiology, the early years of life are assumed to play a crucial role. In addition, there is growing evidence that elucidating the impact of cross-generational effects and epigenetic mechanisms such as DNA methylation can substantially widen the scientific knowledge of the occurrence and progression of these diseases. We are thus aiming at following the course of asthma, allergies, and potential risk factors for their occurrence across three generations by establishing a birth cohort in the offspring of an existing population-based cohort.

Abstract

Background

Asthma is a heterogeneous disease and development of novel therapeutics requires an understanding of pathophysiologic phenotypes. The purpose of the ADEPT study was to correlate clinical features and biomarkers with molecular characteristics, by profiling asthma (NCT01274507). This report presents for the first time the study design, and characteristics of the recruited subjects.

      Abstract

      The mucosal lining of the upper airways represents the outer surface of the body to the ambient air and its contents and is prepared for it as the first line of defense. Apart from the well described physical barrier and the mucociliary clearance, a variety of systems including the airway microbiome, antimicrobial proteins and damage associated molecular patterns, innate lymphoid cells and epithelial derived cytokines and chemokines, and finally the adaptive immune system, and eosinophils as newly appreciated defense cells form different levels of protection against and response to any possible intruder.

Abstract

Asthma is a serious public health problem due to its socioeconomic impact worldwide and affects over three hundred million individuals. This chronic inflammatory airway disease causes hyperresponsiveness, obstruction, increased mucus production and airway remodeling. Due to research advances, our understanding of the pathophysiology of asthma has changed, as has our knowledge of the cells and mediators involved in the inflammation and remodeling that result from asthma exacerbations.

Abstract

The human microbiome can be defined as the microorganisms that reside within and on our bodies and how they interact with the environment. Recent research suggests that numerous mutually beneficial interactions occur between a human and their microbiome, including those that are essential for good health. Modern microbiological detection techniques have contributed to new knowledge about microorganisms in their human environment. These findings reveal that the microbiomes of the lung and gut contribute to the pathogenesis of asthma and allergy.
